Choose 3 Vital Signs
A sign that reminds patients they may not need to step on the scale, and lets them choose which vital signs are taken.

In This Space, You Are…
Signs that affirm patients are safe, heard, accepted, important, valued and seen help foster a space where everyone feels respected and supported.

Patient Name Options
Acknowledging and using patients' preferred names is a vital part of trauma-informed care. Asking for both legal and preferred names shows respect for each person's identity, builds comfort and trust, and still collects essential registration information in a sensitive way.

“Don't Weigh Me” cards
Cards patients can hand to staff to politely request that weight is only measured if medically necessary, opening a conversation about why a measurement might be needed. They support comfort and autonomy, especially for patients who have experienced trauma related to body image or weight.

Past Events & Current Health
A rack card that helps patients recognize why sharing past events with healthcare providers matters, since stress and trauma can be linked with current health concerns.

C.A.R.E. badge buddy
A badge buddy for providers that helps identify ways to be trauma-informed in your role, while signaling to patients that you are committed to C.A.R.E.
